Application in Medical Practice
The Health Risk Assessment enables physicans and other health care professional to automate the task of evaluating a large volume of patient data and preparing customized recommendations. All of the most obvious answers are pre-written. The doctor can use all of his evaluation time to discover the more subtle problems and add them to the recommendations. Click to see a sample report.
The Health Risk Assessment questionnaire addresses topics such as blood results, lung capacity, all major lifestyle factors, personal and family history, symptoms, functional health status and quality of life, health interests, readiness for change, self-efficacy, overall wellness, current fitness level, heart health, stress and coping, eating habits, drug and alcohol use, and medical care. Application in Workplace Medical Claims Reduction Program
The Health Risk Assessment can:
- Provide a comprehensive health and lifestyle assessment based on the most current scientific guidelines for optimal health and disease prevention.
- Help people make healthy lifestyle decisions and make appropriate use of medical services.
- Provide a systematic approach to assessment of an organization's use of medical services.
- Provide information that can be used to design appropriate intervention programs emphasizing modifiable risk factors for disease.
- Provides a way for employees to identify existing healthy behaviors to continue good health practices.
The Health Risk Assessment makes possible these program functions:
- Participants complete the questionnaire, and participate in clinical and fitness testing.
- Participants meet with a health educator to review their health scores in wellness categories. Particular attention is given to scores indicating higher than normal risk factors for disease and referrals to a physician's medical follow-up are suggested when indicated.
- The health educator helps participants to identify and evaluate their readiness for change. The educator also assists individuals to develop simple and measurable objectives to make progress in the change process. Individuals will be encouraged to send a copy of their Physician Summary Report to their Primary Care Provider. Participants will be referred to internal and external health promotion, disease prevention, or disease management intervention programs or services when indicated.
- Recommendations for six month or yearly follow-up Health Risk Assessment is highly encouraged, especially for individuals with high and multiple risk factors.
- Extensive personal, aggregate, and administrative summary reports can be produced with virtually unlimited selection criteria for targeting and subgroup outcomes analysis.
There are three versions of the software:
1. Online version. You will have a branded web page on which your clients or staff can fill out the questionnaire online and get an immediate report that can be viewed online and printed.

2. Manual version with processing service. Machine readable questionnaire is submitted to us for processing. Then the report is mailed to you within a couple weeks.

*NOTE: Processing service price breaks are based on batches sent in per time not per project.
HRA Group Reports: One free Group Statistical Report or Executive Summary Report is free with a minimum of 250 Personal Reports processed in a batch as well as one free for each additional 250 Personal Reports in a batch.
Other Options: Hand entries of clinical data such as blood tests add $0.40/person. ? To have reports mailed directly to participants addresses add $0.60/person for handling plus postage costs that will be billed at current rates set by the U.S. Post Office.
Standard Shipping: For Processing Orders of $100.00 or more, reports are returned in batches by UPS 3-Day Air to your office at no extra charge.
Payment: A PO#, credit Card or Check is required for processing. $20.00 minimum charge.
NOTE: Do not staple, fold. Tear, or affix labels to the questionnaire pages. Age, height, weight and gender responses are required fields on the questionnaires for processing. Check for omitted answers – especially on clinical pages. Average default values will be given if answers are missing.
